Make a Request: 

Steps to a successful Request:

  1. Download the request form.​

  2. Fill out the request form to the best of your ability. The more information the foundation has the smoother the approval process will be.

  3. Email your request form to marygrace501c3@gmail.com

  4. You will receive an email letting you know we received your application.

  5. Approval process takes approximately five days. Upon the boards decision you will receive an email.

Requirements for request:

  • The request must be for the benefit of a patient or family who is dealing with a terminal illness. 

  • We only help with things that a hospice benefit will not cover.  

    • i.e. food, clothing, transportation, etc. ​

  • The recipient does not have to be an active hospice patient.

Non-Pharmacological Interventions

What are some things we offer:

  • Amazon Echo Dot

  • Smart Plug (that is compatible with the Echo Dot)

  • Headphones

  • Essential Oils

  • Diffuser

  • Twiddle Muff

  • Easel Art Set

  • Care for a Caregiver

These interventions must be approved by the Mary Grace Foundation board members. Upon the request for one of these items, please explain why you believe the intervention would benefit the person or family. 

Your request does not have to be for one of the above items, we can also assist with bills, groceries, of set medical payments, other non-pharmacological interventions and more.
